Introduction: Henna, has been applied in eastern cultures for many years, as a hair dye and tattoo. Paraphenylenediamine (PPD) is kind of aromatic amine added to henna. The formulation formed when PPD known be highly toxic. 
 Case report: Here, 16-year-old female patient admitted an emergency clinic with severe angioedema, rhabdomyolysis, acute renal failure caused by after henna ingestion suicidal purposes, was presented. An tracheotomy performed the because respiratory distress, followed up connecting mechanical ventilator intensive care.
 Conclusion: poisoning life-threatening condition, there no specific antidote intoxication. Early intervention essential it life-saving.
